New Year, New Beginning: How To Get The Best Results Whatever Your Goals

It is such a relief to know that every morning we can start fresh with a clean slate and that we can do that any time during the year. A new year is simply a reminder for most people of a promising new beginning. As they say, out with the old (the old that's not working so well, that is) and in with the new.

You have made it past a very busy holiday season. Let us evaluate how you are making time for your own fitness, your own passions and your recovery from those things. It all begins with setting goals and making a decision to work toward them.

Henry Ford had said, "Obstacles are those frightful things that you see when you take your eyes off your goal." Let's look past the obstacles right now and find out: what would you like to do?

Are you balanced in these areas of life: finances, career, social, family, passion, spirituality, relaxation, health/fitness, service to others, self education? If on a scale of 1 to 10 you could score yourself and how much time you spend in each category, then what would that look like? I encourage you to make a list of each life area above and score yourself, 10 means you are heavily invested in a life area, and 1 means you have very little or no investment in that life area. If your numbers are very low in certain areas, then to increase your focus in those categories will help you achieve overall balance and improvement in all areas.

Make a number line for each life area and put numbers one through ten spaced evenly on each line. Then score yourself on each line and see if your zig-zagging all over the place. The lower the number, the more emphasis you need to put on that life area.

Go back to the first question of what you would like to do and apply that to those areas where you are spending the least amount of time. This is where you can see major positive shifts in 2013 toward progress.

If, for instance, you score lowly in the area of health/fitness what are you doing today and what are you willing to sacrifice to help you focus on that very important aspect of your life?

Are you seeking weight loss? Will you choose to integrate walking in your day 6 days per week? If you already do that and it's not enough (because your body is amazing at adapting), then will you integrate 3-4 days per week on interval strength training? How is your nutrition? Be honest. Are you getting 10-13 servings of fresh, raw fruits and vegetables every day? What small steps can you take to work toward that? These are merely examples of assessing your health/fitness life area.

Take small steps in the right direction. Add 20 minutes three days each week of increased exercise or a whole food supplementation to increase your produce. Get these new changes under your belt for three weeks before you integrate a new change (like drinking more water) and turn that into a habit. It takes about 21 days to make a habit of something new.

The biggest mistake you can make is to take on too much too soon and then burn out. Slow and steady wins the race.

There are four basic "rules" to weight loss that go beyond simply eating less and exercising more:

Stop smoking  

As simple as it sounds a modern approach to weight loss is to stop smoking. Stopping smoking can be one of the best decisions you make to improve your health as it is associated with cancer, especially cancers of the mouth, oesophagus and lungs and it accelerates aging.

There is some evidence to suggest that if you quit smoking you are more likely to put on weight - and this may be because nicotine acts as an appetite suppressant / or you just tend to eat something when you would have had a cigarette - but this certainly doesn't mean that you should smoke to lose weight. But there is other research that refutes this claim.
In fact researchers have shown that females who smoke 10 cigarettes a day as teenagers are more likely to be overweight as adults, this trend didn't apply to males in this study, but in young males smoking tends to accompany alcohol and alcohol as we will see also contributes to obesity.

Reduce and modify alcohol intake

Alcohol contains an abundance of calories that often don't get accounted for when you are trying to lose weight. However a modern approach to weight loss is to modify your alcohol intake rather then quit all together.

The best wines to drink are organic as the grapes haven't been sprayed with pesticides, but also try and pick wines rich in a compound called resveratrol which has many health benefits. These include pinot, merlot and all Spanish wines.

Exercise  

There is no doubt that exercise can help you lose weight and that not doing any exercise can contribute to you being fat. But there are many types of exercise - yoga, Pilates, bootcamps, running, lifting weights or simply walking the dog - and you need to find what is right for you.

From an evolutionary perspective we are designed to be highly active, not designed to sit at desks all day and drive or take the bus everywhere we go. We can get a good idea of how much activity we need to get each day by looking at isolated indigenous communities that still live a very basic lifestyle. It is estimated that these communities may "exercise" in terms of hunt, gather, clean, build shelter, play etc... up to 5 hours a day which seems quite a lot and certainly a lot more than modern Westernised humans get - but just think they get 19 hours a day to sleep, eat, relax and have fun. Wouldn't that be great?

Most people think that "cardio" be it running, or using the rower or the cross trainer in the gym is the best way to lose weight, and there is no doubt that doing some cardio can help.

But a modern approach to weight loss includes the need to build muscle as well and this involves doing some resistance training. As with cardio, resistance training comes in different forms, be it using your body weight, using a TRX or a Swiss ball, kettle bells, dumbbells or resistance machines. I think that all of these things can be used for effective weight loss and they will certainly add variety to your exercise regimen, but the true secret to success is to change things often to prevent your body from adapting. This might take the form of doing cardio for a month, then using TRX or Swiss balls for a month, then doing Pilates or boxing for a month then doing some weight training - just keep changing things up.

The other secret to success, and this hold true for success in any other industry or in life, is that you need to work hard at what you are doing. If you go to the gym and sit on one of the bikes watching the TV or reading a book you are not working hard enough to get results.

Eat a low glycemic load diet 

This is a modern approach to weight loss that involves consuming healthy protein (such as meat, fish, seafood, poultry and eggs) and fats (such as nuts, seeds, olive oil, avocados, oily fish and coconut oil) with low glycemic load fruits, vegetables, grain and legumes.

The glycemic load is a system of classifying carbohydrate containing foods by how quickly the carbohydrate is released into the blood stream. The GL system scores foods as low GL that are less than 10, moderate GL that are 11-20 and high GL as anything over 20.
It is beyond the scope of this article to outline the GL of all carbohydrate containing foods, but a general rule is that thin skinned fruits such as berries, apples, pears and plums and green leafy vegetables are all low GL. Things like potatoes, bread and whole grains are moderate GL and all processed foods such as sugar, sweats, chocolates and white grains are high GL. This may be a bit of an over simplification but you will certainly start losing weight if you eat mainly low GL food, a little moderate GL food and no high GL foods.

Advanced topics for weight loss

There are many people who will follow this advice and who still struggle to lose weight. If you are one of these people you'll need to think about the following four contributing factors for a truly a modern approach to weight loss:

Gut microbiota  

Inside each of us are trillions of cells of bacteria, with up to 2 kg of bacteria living in our gastrointestinal tract known as the gut microbiota. It is believed that the bacterial cells inhabiting our gut outnumber the cells that make up our own body by a magnitude of 10:1! Are you really who you think you are?

High fat low fibre diets alter the gut microbiota leading to an environment that creates lowered gastrointestinal (GI) colonisation of healthy bifidobacteria, this in turn leads to GI inflammation, intestinal permeability and the leakage of lipopolysacchardies (LPS) into the blood stream. It has been shown that LPS creates an immune response in the body that increases the storage of fat, creates inflammation and decreases insulin sensitivity. All of this contributes to being overweight.

If, however, you take prebiotic fibre and probiotics such as bifidobacteria this decreases the releases of LPS into the blood improving insulin sensitivity, decreasing inflammation, decreasing fat storage and decreasing appetite. Simply taking a probiotic supplement and eating prebiotic foods is a modern approach to weight loss that can help you.

Vitamin D   

Vitamin D is only really obtained by direct exposure to sunlight. Over 3000 genes are affected by vitamin D and beyond the role vitamin D plays in bone and immune health it plays a huge roll in cell signalling and cell differentiation. We know from research that vitamin D deficiency negatively affects insulin sensitivity / glucose tolerance and leads to type 2 diabetes and type 2 diabetes is associated with being overweight.

So a modern approach to weight loss is something as simple as making sure your vitamin D levels are up to par. This can help improve insulin sensitivity and aid having optimum body composition. You can ask your doctor for 25(O-H)D3 test and supplement accordingly based on the results. A baseline of 2000-500IU is a good place to start.

Methylation  

Methylation is a process that happens in many body tissues such as the brain, the liver, the lungs and the heart. It is a set of chemical reactions that turns methionine into something called SAMe. The process requires amongst other things B12, folic acid and B6. Due to our unique genetic make up some of us require much more of these B vitamins than others and if we are deficient we get a build up of homocysteine in the blood. People who have higher homocysteine in their blood are much more likely to suffer from all manner of diseases as well as be overweight.

Research done in rats shows that the rats that are hypomethylators (low in B vitamins or have a genetic fault requiring higher levels of B vitamins) are overweight and obese. You can have your homocysteine measured by a simple home blood test kit available from York Test, after which you can supplement with the active forms of B12, folic acid and B6, which is truly a modern approach to weight loss.

Toxins  

Thousands of chemicals have been released in to our environment in the past 50 years, many with no knowledge about they will affect human physiology. They range from plastics such as dioxins and phthalates, to pesticides, fungicides, and herbicides as well as heavy metals. Many of these chemicals have an oestrogenic effect in our body and have been termed obesogens by functional medicine experts.

These chemicals basically screw up human physiology causing DNA damage, depleting and displacing nutrients from the body, altering cellular communication and energy production and placing a toxic burden on the body.

Identifying and eliminating these toxins is beyond the scope of this article but you can reduce your exposure to these chemicals by buying organic food. The Number One Weight Loss Factor

There are thousands of reasons people struggle to lose weight, but at our personal training studio in Chiswick, one reason stands head and shoulders above all others. I'm talking about blood sugar and insulin resistance.

Although most people know that blood sugar and insulin plays a part, how many of us really know what blood sugars role in causing weight gain really is. Why does blood sugar becomes too high, why does it get too low, why is this important and crucially, how we can regulate it. This an extremely complex and vast topic but I'm going to simplify the subject as much as possible.

BLOOD SUGAR EXPLAINED

Whenever we consume any form of starch or sugar carbohydrate, this becomes available as blood sugar, otherwise known as blood glucose, in the bloodstream. How quickly it converts to blood glucose depends on the glycemic index of the carbohydrate. Glucose itself would have a profile of 100 which means it is all instantly available as blood sugar whilst lentils can have profile of as little as 21 which means the carbohydrate becomes available as sugar more slowly.

When our body detects blood sugar, it releases a hormone from the pancreas called insulin. This increased level of insulin in the bloodstream enables energy from glucose to become available to the muscle and liver cells as energy; see insulin as a key that unlocks the cells to allow the glucose in.

So, if a person who naturally releases too much insulin, eats a high glycemic food like pasta, they will have an oversupply of the hormone. If this same person were to eat a low glycemic carbohydrate like lentils, they would have a much better chance of releasing the sugar more evenly into the blood stream as the insulin released would be lower.

It is important to note that Muscles and the liver only need enough glucose to replenish the stores, and liver stores glucose as glycogen which it makes available in times of crisis. An over production of insulin makes all the glucose instantly available but the muscles and liver cells will only take what they need.

Fat cells, on the other hand, are eager recipients of any blood sugar made available to them by excess insulin, consume the remainder up and thus expand. Fat cells are essentially our emergency stores of energy, for when we run out of food, but only once all the glycogen stores are gone from the liver. As you will see, that's unlikely to happen when we continue to eat refined carbohydrates.

Now, the real problem that has been caused by eating a high glycemic carbohydrate becomes evident. The energy from the pasta has now been utilized by the greedy fat cells, so your blood sugar has dropped too low, the insulin spike caused all of the sugar to become available at once and there is none left.

Your liver is stubborn and doesn't like giving up its stores unless it has to, but we need energy on a constant basis, even while sleeping, and as we discussed your fat cells are not giving up a bean of energy until all the glycogen is gone from your liver.

So your body is faced with a dilemma, it needs some more sugar urgently but the liver is unwilling to release any and the fat cells won't be releasing any until the liver is depleted of glycogen stores. The message is sent to your brain loud and clear. You are hungry. EAT! But you only just ate, how can it be possible that you're hungry again.

Not only has the pasta potentially increased the size of your fat cells, it hasn't even sorted out your hunger levels, because the glucose is no longer available to the muscle cells and liver which need a slow but constant supply. The fat cells had all the glucose and because you're hungry you eat a source of carbohydrates which relieves the hunger quickest. A starchy or sugary carbohydrate. It's a catch 22 in the very truest sense.

This internal message to raise the blood sugar is so overwhelming that seldom will we manage to overcome the craving.

WHAT CAN YOU DO ABOUT IT?

Some people release less insulin naturally and so can tolerate higher glycemic foods, meaning a larger consumption of carbohydrates is not such a problem, in others it can have catastrophic effects on their body composition.

Everybody suggests working on the glycemic index, which is great for some people, but for others the fat cells have grown to such a point where the insulin keys no longer fit the cells to receive the sugar, which is essentially type 2 diabetes.

The ultimate goal is to not have any fluctuation in blood sugar at all.
Think about this for a second. We do all need carbohydrates if only for brain function, anybody who tells you otherwise is probably trying to sell you something. Fortunately, we know that carbohydrates come in 3 forms.
  1. Sugar - which we need to avoid at all costs.
  2. Starch - Although it varies on the GI scale, ultimately becomes glucose which will cause some insulin release and therefore blood sugar variations
  3. Fibre - Which has no impact on blood sugar whatsoever.
What I believe to be the most effective strategy in those with significant blood sugar related weight issues and certainly in type 2 diabetics, is a diet which is very high in fibrous, low starch vegetables. I will often suggest carbohydrates come almost exclusively in the form of green cruciferous vegetables in the early phases of a dietary regime.

Of course, if you are going to take this approach you will also need to consider your intake of good quality fats as an alternative fuel source, but that's a whole new conversation.

In taking a less extreme path towards long term blood sugar balance for those with less pressing blood sugar issues, we can implement a simple intelligent carbohydrate 'swapping' strategy.

Eat only whole natural carbohydrate sources like sweet potato, brown rice, quinoa and lentils in place of white rice, ALL bread and pasta.

Eat fruit as a carbohydrate with some natural Greek yogurt instead of desserts like chocolates and ice creams.

If you really do struggle with complete elimination of refined sugars from your diet then the only instance when eating foods high on the glycemic index could be considered a good idea is post exercise, where the spike in insulin can be beneficial as your muscle cells are more agreeable to receiving the available blood sugar to replenish and repair.
